I consent to emergency medical treatment for my child.

I will supply sunscreen and/or insect repellent for my child.

I understand that my child may not attend camp if he/she has had one or more of the following symptoms: Fever over 100.4, vomiting, diarrhea, undiagnosed rash, has started antibiotics within the past 24 hours, congestion/coughing. we kindly ask that you keep sick children at home for a minimum of 24 hours.

I understand that I am responsible for ensuring my child is covered by health insurance and that any injury or illness incurred at camp in the financial responsibility of the parents and not that of Quantum Youth.

I give permission for my child's photograph to be used on social media or for advertising purposes.

Arrival/Dismissal: Please do not arrive at camp earlier than your scheduled drop off time and implemented curbside drop off and pick up procedures. Please be prompt when picking up your camper.

I give permission for my contact information to be shared with other camp parents.
